Boy Parts by Eliza Clark - she’s Patrick Bateman in the form of a hot girl. She’s an artist from Northern England who takes photographs of ugly men. Her inner monologue is brutal but hilarious.

Sticking with horror, try some Thomas Ligotti stories. They all fit the bill to some extent, but The Chymist jumps out as an experimental piece that does this super well--it is written as only the dialogue of a killer, with all narration and his victims responses omitted.
Read Molloy by Beckett. It’s the epitome of unreliable narration. Failure to tell a reliable story, indigence of all knowledge, and a general inability to know what one is saying despite wanting to say it was Beckett’s conceptual rejection of Joyce, and he does it well. It’s bleak and funny

no longer human by osamu dazai. i remember reading i could never tell how the narrator was actually perceived by the world around him because he would say one thing of himself but the events that occur subtly told the opposite. i actually need to re-read this book because the ending left me feeling hollow and shocked in a good way.
Tampa by Alissa Nutting is a fucked up female version that deals with a pedophile. The main character from Hunger by Knut Hamsun isn’t all there but I can’t remember if he narrates the book or not, it’s been a while.
Perfume: The Story of a Murderer by Süskind is good, so is The Piano Teacher by Elfriede Jelinek.
Just one mentally unstable person? Nearly every character in The Sluts by Dennis Cooper is borderline psychopathic.
